VALMAR SUPPORT SERVICES

Community Aged & Transport Services

We provide assistance to enable older people to live independently within their own home and community.

Our specialist services are individually designed with you to ensure independence, health and wellness. You may be eligible to receive assistance if you are:

• An older person aged 65 years or over and having difficulties managing your own home or over 50 if you identify as an Aboriginal or Torres Strait Islander person.
• A carer providing help to an older person aged 65 years or over.
• If you have been allocated a Home Care Package.

Cost of Service

The Commonwealth Home Support Program is supported by the Australian Government.  Client contributions are a condition of service provision with all contributions supplementing funding and allowing the service to have greater capacity to assist and expand our aged care service provision with the communities we deliver services.

Home Care Packages

Costings are charged by the hour depending on your individual need and Home all associated fees align with all departmental funding requirements.  Our price list for the delivery of Home Care Package services is available on My Aged Care and on the Valmar Website or visit our friendly team in our office in Lockhart for any assistance.

Our Local Team – Lockhart and Districts

Our Aged Care Services team is located in our Lockhart Office with all of our staff having broad local knowledge of the area and any community based support services that may assist you in achieving your goals.

Services We Provide:

Community Transport

Is a quality specialist transport door to door service.  This service may include but is not limited to providing transport to medical appointments, to access the community, to conduct personal business and shopping, and other social outings for groups and/or Individuals to and from various locations within and surrounding Lockhart and Districts.  

Individual & Group Social Support

Social Support enables your independence ensuring access to your community and your social networks.  

Meals

Both hot and frozen meals delivered to your door depending on your requirements.

Flexible Respite

Respite is designed to be responsive to the needs of the carer.

• In Home Respite
  • In Home Respite services will be provided in the comfort of your own home, on a one-on-one individual basis allowing carers to have regular breaks.
• Centre Based Respite
  • Allows you to get together to enjoy the company of others, a meal, fun outings, recreation, and leisure activities that fit with your interests.

Domestic Assistance

Is assistance to enable you to complete the household tasks you find difficult.  This may include, but is not limited to, making/changing beds, mopping, vacuuming etc.

Personal Care

Our caring support staff can assist you with your daily self-care.  You decide how much or how little care support you need.

Home Maintenance and Modifications

Specialises in maintenance and modifications of your home or yard.  The purpose of the service is to make the home safer and more accessible.  This service can include tasks such as installation of rails and lawn mowing.
